UHC Pharmacy Care Hub (formerly Telehealth)

Program Overview

updated 3.14.25

Announcements:

  • UHC Pharmacy Care Hub (formerly Telehealth) has relaunched with some updates! There are updated resources below, including an updated implementation guide, relaunch webinar recording, and slide deck.
    • This updated implementation guide helps to focus on what services pharmacies can provide eligible patients, without necessarily connecting the patient to additional services, via telehealth (although that is still available).
  • Each month with the patient list, the statewide eligibility file will be attached so that you may look up patients by their Medicaid number to determine if they are eligible for services that month (even if they are not on your targeted list of patients).
  • We have learned that Injection of therapeutic agent (28289002) has been removed from the snomed code international library. Some Technology Providers have in turn already started removing it from their systems.  For this snomed code, Administration of Medicament (18629005) has been accepted by the payor as an option that can be used instead of the older, now outdated, 28289002 code.
  • Renewed SOW with updated pricing in the fee schedule has been added below under Resources > SOW.

FAQ: How do I update the point of contact(s) who receive a patient list for the UHC Telehealth program for my pharmacy NPI? The best way to update the contact who receives patient lists for the UHC HRA program is to re-complete the program enrollment form. This can be done for the pharmacy, and all contacts who desire to receive the list should ensure their information is included in the enrollment (can separate with a comma if multiple). Access the CPESN Program Enrollment GatewaySelect Program 52: United TeleHealthcare. You will then receive an email with instructions on how to finish updating the pharmacy's enrollment.
